- Complete the authorizations records request form online.
- Print and fax it to us at (626) 446-8952 or you may email it to: medicalrecords@cmbsllc.net.
- We will mail your records to you; please allow 5-7 business days.
FOR URGENT REQUESTS, please call us at (626) 203-4924 to arrange pick up within 1-2 business days. You may pick up records from our Arcadia business office.
